Polydactyly is a prevalent birth anomaly observed in the foot, and it is described as preaxial, central and postaxial based on locations. Postaxial polydactyly ( fifth toe ) is the most common type. Classification system guides treatment options. Clinical presentation is diverse and surgical treatment needs to improve appearance, alleviate pain, normalize footwear and solve psychological problems. Excising the supernumerary digit, with subsequent careful reconstruction, should be the mainstay of treatment. Careful consideration and individualized planning of surgical treatment are required to obtain the best possible outcomes.
