Objective To present the diagnosis and treatment of acute rupture of the long head of biceps brachii. Methods A retrospective analysis of one patient with no typical clinical manifestations was conducted. Results A 50 year old female patient complained "sudden left upper arm swelling and blood stasis for 1 day". After detailed history, medical examination and complete auxiliary examination, acute rupture of the long head of biceps brachii was diagnosed. After 1 month of conservative treatment, popeye sign is positive. And acute rupture was confirmed. Conclusions Acute rupture of the long head of biceps brachii is not common in clinic. Misdiagnosed and missed diagnosis are easy to occur because its clinical manifestations are not typical. This case is worth learning.
