Total hip/knee arthroplasty is an orthopedic surgery with significant fibrinolysis and inflammatory system activation. However, the perioperative use of tranexamic acid (TXA),a synthetic lysine derivative, in total joint arthroplasty (TJA) greatly reduced the total blood loss and decreased the transfusion rate without increasing the risk of thromboembolic complications. Traditional dexamethasone was used to inhibit perioperative inflammations. Several studies in recent years have shown that TXA can not only inhibit fibrinolytic reactions after TJA, but also have obvious anti-inflammatory effects. The aim of this review is to discuss the relationship between flbrinolysis and inflammatory system, the latest progress, potential problems and orientation of future research on the anti-inflammatory effects of dexamethasone and TXA in TJA.
