Thoracic lumbar fracture and dislocation is one of the most common injuries of the spinal cord. In order to correctly understand and handle this problem, international spine surgeons carry on the unremitting exploration by the analysis of its anatomy, biomechanics, image for decades, and successively established several international thoracic lumbar spine injury classification systems, especially new AO thoracic lumbar spine injury classification system. With the evolution of these theories, surgeons have greatly improved the clinical diagnosis accuracy and reduced the misdiagnosis and mistreatment rate, improving the medical level significantly. However, due to the different mastering of theories and techniques, the lack of understanding on the classification system, misdiagnosis and mistreatment still exist. Therefore, it is necessary to review the characteristics of the thoracic lumbar fractures again, especially the new progress of the clinical classification and treatment.
