A posterior lateral approach was used for focal debridement in 22 cases of tuberculosis of the lumbar spine; the lesion is explored from the lateral aspect of the lumbar spine after separation of the peritoneum in front of the transverse process. when a sinus is present over the back, one may follow its path around the transverse process and reach the site of the lesion. This approach is good enough for exposure of as high as T
6 down to L
5. 11 of the patients had gibbus, 12 had abscess and sinus over the back. 18 cases were followed for more than one year; the results were all satisfactory.
